Management and Treatment of Obesity in Primary Care
An Evolving Landscape
This free CME program from NEJM Group explores the latest evidence on lifestyle, pharmacological, and surgical interventions, as well as recent progress in understanding the causes of obesity. Consisting of four distinct learning opportunities, each activity aims to equip primary care clinicians to navigate the complex, evolving landscape of obesity management.
CME/CE Program Overview
- Examine the roles of environmental and genetic factors in obesity.
- Articulate the mechanisms and pathophysiology of obesity.
- Review common risk factors and comorbidities associated with obesity.
- Assess multiple factors that influence the prevalence of obesity among diverse populations.
- Evaluate the evolving landscape of nutrient-stimulated hormone-based treatment options for obesity, and management of common side effects of these medications for your clinical practice.
- Apply knowledge of current best practices in obesity management to confidently manage patients with obesity.

In a trial in patients with cardiovascular disease and overweight or obesity but no diabetes, semaglutide was superior to placebo in lowering the risk of major adverse cardiovascular events at a mean follow-up of 39.8 months.
In patients with heart failure with preserved ejection fraction and obesity, semaglutide (2.4 mg) led to greater reductions in symptoms and physical limitations and greater improvements in exercise function than placebo.
Management of obesity in adolescents should routinely include intensive treatment with regard to lifestyle and diet, with antiobesity medications and bariatric surgery considered if indicated.
In this trial, adolescents with obesity assigned to weekly subcutaneous semaglutide plus lifestyle intervention had a greater reduction in BMI than those who received lifestyle intervention alone.
The authors review the many adverse effects of obesity on pregnancy, including hypertension, diabetes, insulin resistance, subfertility, miscarriage, and congenital abnormalities.
In this randomized trial, adults with obesity treated with weekly tirzepatide, a glucose-dependent insulinotropic polypeptide and glucagon-like peptide-1 receptor agonist, had major weight loss over 72 weeks.
This trial evaluated the efficacy and safety of time-restricted eating as compared with daily calorie restriction for weight loss at 1 year among patients with obesity. Time-restricted eating did not produce additional benefits with regard to the reduction from baseline in body weight, body fat, and metabolic risk factors as compared with daily calorie restriction.
This double-blind study randomly assigned adults with obesity (or overweight and with one or more weight-related coexisting conditions) without diabetes to 68 weeks of once-weekly subcutaneous semaglutide (2.4 mg) or placebo, plus lifestyle intervention. Semaglutide therapy was associated with sustained, clinically relevant weight reduction.
This study compared long-term mortality and life expectancy among patients who underwent bariatric surgery or received usual obesity care in the Swedish Obese Subjects study and in persons in the SOS reference study. The adjusted median life expectancy with surgery was 3.0 years longer than in control patients but 5.5 years shorter than in the general population.
Evidence is accumulating that eating in a 6-hour period and fasting for 18 hours can trigger a metabolic switch from glucose-based to ketone-based energy, with increased stress resistance, increased longevity, and a decreased incidence of diseases, including cancer and obesity.
The authors report 12-year follow-up results of Roux-en-Y gastric bypass versus no surgery. The results show long-term durability of weight loss and effective remission and prevention of type 2 diabetes, hypertension, and dyslipidemia after Roux-en-Y gastric bypass.
In a study of diet and several exercise modes to reverse frailty and prevent weight-loss–induced reduction in muscle and bone in obese older adults, weight loss plus combined aerobic and resistance exercise was the most effective method of improving functional status.
Obesity is prevalent in the U.S. population and contributes significantly to morbidity and mortality. Treatments include behavioral therapy, pharmacotherapy, and bariatric surgery. Some sequelae of obesity are reversed with weight loss. Maintaining weight loss is a challenge.
